RELEASE AGREEMENT

I,____________________________________, as breeder/owner of sire, release this dog to USKBTC Rescue.

USKBTC Rescue assumes full responsibility for the dog at the time of this transaction. At this time the breeder/owner of sire relinquishes all responsibility for this dog.

USKBTC Rescue will treat this dog according to it’s established procedures for placing dogs. I authorize the USKBTC to euthanize this dog if it is deemed necessary due to health issues or unstable temperament.
